Job Summary To promote resolution of social and emotional problems which are or are expected to be an impediment to effective treatment of the patient's medical condition or rate of recovery through direct and indirect services.
To provide assistance in dealing with social and emotional problems through evaluation, on-going assessment, counseling and referral to appropriate community agencies.
Accountabilities u00B7u00A0u00A0u00A0u00A0u00A0u00A0u00A0 DOCUMENTATION RESPONSIBILITIES include:
u00A0 completes MSW assessments in a timely manner per agency policy and procedure; documents identified problems, strengths, interventions and progress appropriately in the clinical record; completes recertification and discharge summaries in a timely manner; meets productivity expectations; provides office staff with appropriate reports and documentation in a timely manner per agency policy and procedure; and, turns in weekly schedules and payroll sheets to supervisor according to agency policy.
u00B7u00A0u00A0u00A0u00A0u00A0u00A0u00A0 TEAM RESPONSIBILITIES include:
u00A0 consults with other disciplines regarding clinical work with the patient and caregivers; participates in staff development and training; endeavors to work cohesively with all members of the Home Health team to promote good working relationships; participates in clinical team meetings, utilizes lines of authority appropriately, willingly accepts and completes administrative duties as assigned; provides a positive image of PH Home Care and acts as a community liaison betweenu00A0PH and the community; and, serves on agency committees as assigned.
u00B7u00A0u00A0u00A0u00A0u00A0u00A0u00A0 DIRECT PATIENT SERVICES include:
u00A0 assesses patients referred for MSW services in a timely manner per agency policy and procedure; develops an appropriate and goal-oriented plan of care in a timely manner per agency policy and procedure; involves the patient and family members in development of the plan of care; informs the patient and family about and facilitates referrals to appropriate community resources; provides appropriate counseling and intervention to patients and families as necessary; observes progress made and alters plan of care based on changing patient needs; reports changes in patient's condition to the case manager and physician as deemed necessary; implements infection control and safety measures per agency policy and procedure; and, provides discharge planning services as indicated.
u00B7u00A0u00A0u00A0u00A0u00A0u00A0u00A0 PROFESSIONAL RESPONSIBLITIES include:
u00A0 pursues professional development opportunities; shares knowledge with other team members through informal and formal means; adheres to agency policies and procedures; participates in the annual performance evaluation by undertaking self-evaluation and establishing personal goals for the following year; and, maintains licensure by fulfilling all requirements.
Supervisory/Management Responsibilities This is a non-management job that report to a supervisor, manager, director or executive.
Minimum Requirements Education - Master's degree in Social Work Experience - 1 year of experience as a Social Worker in a healthcare setting Required Certifications/Registrations/Licenses Licensed Social Worker in South Carolina Valid driver's license, acceptable MVR Current auto insurance Other Required Skills and Experience Reliable transportation Ability to perform home visits and asses patient environment Driving is required for this position using the employeeu2019s personally owned vehicle.
u00A0 Employee must have reliable transportation and will be expected to adhere to the Prisma Health Driver Safety Policy and specific department driving policies.
Ability to work independently, make accurate, and at times, quick judgments.
Ability to respond appropriately to crisis outside of a hospital setting.
Acceptance of and adaptability to different social, racial, cultural and religious modes.
